Creating a system-wide healthcare model leads to patient-centered care
Centura’s vision is to transform the future of healthcare for its patients and communities. To help Centura achieve a systemwide model for ambulatory neighborhood health centers, ERDMAN worked to:
- Standardize the operational model among Centura’s practices and facilities
- Create a consistent brand presence
- Shift Centura’s focus from sick care to wellness care
To establish its new health model, Centura selected ERDMAN to develop an overall strategic plan, submarket definition, and location-specific deployment plan. The work also included creating an operational and experiential model for care, as well as facility design standards. As a result, Centura became equipped with a playbook for implementation that unifies its brand across multiple markets and emphasizes personalized, team-based attention and whole-person care.
Planning and implementation go hand in hand. As we worked with ERDMAN from up-front planning and understanding the market needs to design and implementation, we were able to work with the same team. This ensured everyone understood the overarching design goals so that early guidelines were established into our Neighborhood Health Centers.
