Founded in 1982 Access Community Health Center is a Federally Qualified Health Center offering primary, dental, and behavioral health care, as well as pharmacy services and resources for housing, food assistance, and employment counseling to more than 30,000 individuals across southwest Wisconsin. In planning for the replacement of their anchor south side clinic it was critical the following be achieved:

Improve operational efficiency while substantially expanding capacity

Developing an environment and experience that drives recruitment and retention

Maximize return on investment while maintaining a conservative budget

Demonstrate active community engagement through compassion, inclusion, stewardship and equity.

Access chose ERDMAN to plan and design a facility that provided a new approach to care with a patient-centered medical home model and an integrated care team concept. Our planning and design teams engaged with the Access Patient Advisory Council, providers, and staff to understand needs while simultaneously evaluating processes and opportunities for efficiency throughput to serve more patients and broaden access to exceptional care.
